Application Report	May 12, 2008	$1,750 - small organizations $2,250 - medium organizations $2,750 - large organizations
Site Visit	Visits will be conducted September 8-26, 2008	Vary according to number of sites visited, number of examiners and duration of visit. Not expected to exceed $4000 for small and medium organizations
