If you’d like to attend the luncheon, it’s an additional $35 per person.
AFTER November 16th, registration is $225 per person (lunch is additional).

Substitution/Cancellation Policy: We understand schedules change, so the OQF will accept substitutions at any time using the Substitution form. A substitution fee of 50 dollars will be charged for each substitution made. If a substitution is not available, requests for refunds must be made in writing to the Oklahoma Quality Foundation on or before November 15, 2012. A 20 percent cancellation fee will be charged against the entire registration amount for every cancellation received by November 15, 2012. All cancellations must be in writing, and all refunds will be issued by check after the conference. No refunds will be considered after November 15, 2012 for any portion of the conference. By initialing here, I acknowledge that I have read, understand, and agree to be bound by this substitution/cancellation policy.
